ILYouth2 Youth Registration
& Liability Release!

Please note:

• Registration and liability release must be completed by a parent or legal guardian.

• Responses to the registration form will be viewed by ILYouth2/ILY2 LLC staff only.

• If you are registering multiple campers, you must fill out individual forms for each camper.

• We may ask families to repeat this form from time to time, to ensure we have up-to-date information.

This is required for youth to participate in programming at ILYouth2. Youth under 18 are not able to be dropped off at the ILYouth2 studio (925 NW Flanders) unless we have this registration form and liability release on file for them.
